Job Description:
Nationwide Therapy Group is seeking a travel nurse RN Case
Management for a travel nursing job in Tuba City, Arizona. Job
Description & Requirements
Shift: Monday to Friday, 8:00 AM - 5:00 PM, with weekend rotation
and call
Location: Tuba City Regional Healthcare, serving Navajo, Hopi, and
Southern Paiute tribes in northwestern Arizona
Position: Outpatient/Inpatient Social Worker/RN Case Manager
Start Date: TBD (housing and travel must be secured)
Duration: 12 weeks
Required Qualifications:
Fingerprinting required and cleared prior to start
Current address for fingerprint packet submission via Federal
Express
Current immunizations (including MMR, Tdap, Flu shot, negative Tb
skin test from the past 2 years)
Clean background check and negative 10-panel drug screen (within 7
days)
Must have RN license (not necessarily AZ state)
BLS Certification (American Heart Association)
Minimum 3 years of experience in clinical care settings
At least 1 year of travel experience
Experience in discharge planning, case management, or utilization
review
Knowledge of electronic health record systems
Bachelor's in Nursing (must obtain within 2 years of hire)
Preferred Qualifications:
Experience working with Native American populations
1 year of Case Management experience
Key Responsibilities:
Supervise and support high-risk patients
Design and manage a continuum of care
Collaborate with multidisciplinary teams (e.g., providers, nurses,
social workers)
Perform utilization review of inpatient/outpatient cases
Link clients to appropriate community resources
Ensure the efficient use of healthcare resources and enhance
patient care
Report to the Director of Case Management/Care Coordination
Additional Information:
Orientation: Every two weeks
Housing Application: Must be submitted 2 weeks before the start
date
COVID Exemptions: Only medical exemptions considered (must be
approved by medical review board)
