• Certification Details
• Basic Life Support (BLS) certification by the American Heart Association.
  • Job Details
  • Incumbent serves as an Outpatient/Inpatient Social Worker/Registered Nurse (RN) Case Manager for TCRHCC and Sacred Peaks Healthcare Center (SPHC) and is responsible for direct supervision and supportive contact for the specific group of identified high-risk patients.
  • Responsible for designing and managing a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency.
  • Responsible for assessment, service planning, and resource acquisition, monitoring progress and initiating and responding to emerging client needs.
  • Responsible for utilization review of patient cases in the inpatient and outpatient services departments.
    • Job Requirements
    • Associates degree in Nursing, must obtain bachelors in nursing within 2 years of hire.
    • Must have a minimum of one (1) year of experience in the clinical care setting.
    • Experience and knowledge of discharge planning, case management, or utilization review processes.
    • Demonstrate knowledge of electronic health record systems.
      • Schedule Information
      • ORIENTATION: Every two (2) weeks only.
        • Unit Specific Information
        • The nursing staff includes registered nurses, licensed practical nurses, nursing assistants, community health nurses, nurse practitioners, nurse midwives, CRNA, and community health medics.
          • Additional Details
          • COVID EXEMPTIONS: We do not accept religious exemptions. If a candidate submits for a medical exemption, we will have to get approval from our medical review board to proceed with onboarding.
